Furusato One-Stop Form (Wanstoppu Tokurei Shinseisho) — How to Fill It In and Which ID to Send

What furusato tax (ふるさと納税, furusato nozei) is and where foreign residents get caught is covered in Can Foreigners Use Furusato Tax? How Not to Lose Money. This article is the next step — what you need once you are actually looking at the form.

Furusato portals are effectively Japanese-only. A few have added English or Chinese, but item descriptions there are machine-translated, and none support most other languages. So people understand the scheme and then stop, because they cannot tell which button to press.

1. Japanese words on the donation screen

🔴 One box you must not miss

At the end of the donation form there is a line like this:

ワンストップ特例申請書の送付を希望する (wanstoppu tokurei shinseisho no sousou wo kibou suru — "please send me the one-stop exemption form")

Tick it and the municipality posts the form to you. Miss it and nothing arrives; you have to download and prepare the form yourself later. It takes five seconds at the time of donating.

2. The form, field by field

The document that arrives is formally called 「令和○年寄附分 市町村民税・道府県民税 寄附金税額控除に係る申告特例申請書」. It is a standard form set by the Ministry of Internal Affairs and Communications, so the fields are identical everywhere.

3. 🔴 Two checkboxes — they are the whole document

Near the bottom sit two boxes. The ministry's form states it plainly:

申告の特例の適用を受けるための申請は、①及び②に該当する場合のみすることができます。 ("This application can be made only where both ① and ② apply.")

Both must apply, and both must be ticked. If either is blank, the application does not stand.

① 申告特例対象寄附者である — are you someone who does not need to file a tax return?

If you are an employee whose employer handles year-end adjustment, usually yes. You do not qualify if:

② 五つ以下である — did you donate to five municipalities or fewer this year?

Several donations to the same municipality count as one. The moment a sixth municipality is added, one-stop becomes void for all of them and you must file a tax return instead.

⚠️ If you send in five forms and then donate to a sixth municipality, the first five die too. Count ahead if you plan to add more.

4. Identity documents

You enclose copies of identity documents with the form. The combination depends on what you hold.

A residence card (在留カード) or special permanent resident certificate counts as proof of identity. Several municipalities list them explicitly. If you only have documents without a photo (health insurance card and similar), you need two of them.

⚠️ Careful with the notification card. If your details changed — you moved, you married — and the card was never updated, it no longer works as identity verification. Moving is common enough that this catches people.

⚠️ Accepted combinations differ slightly between municipalities. The table above is the general rule; check the leaflet your municipality encloses.

5. Where foreign residents get caught

The address must match your resident record. If you filed a change of address but never updated the back of your residence card, the two will disagree. Write the resident record address.

Match the name spelling to your resident record. Mixing the romanised passport spelling, a common-use name (通称名) and a katakana rendering makes verification fail.

Online filing needs the My Number Card itself. It works by reading the card's IC chip for public key authentication, so a notification card cannot be used online. Post the form instead.

If you will not be in Japan on 1 January of next year, this year's donation will not produce a deduction. Enter your departure date in the ceiling calculator and it will tell you.

6. Deadline

It must reach the municipality by 10 January of the year after you donated — arrival, not postmark. Donations made late in December leave very little room, so act as soon as the form arrives.

If your details change afterwards (a move, for instance), file a 「申告特例申請事項変更届出書」 by the same 10 January.
